What is a Hospitalist?
A hospitalist is a medical provider who specializes in the care of hospitalized patients. They know the hospital environment and provide personalized care tailored to your unique needs. Working in the hospital, they are available 24 hours a day.
Hansen's hospitalist is committed to providing you with quality care during your stay at Hansen Family Hospital.
Why A Hospitalist?
Because a hospitalist practices within the hospital, they can promptly follow up on diagnostic tests and adjust your treatment as necessary. They are on-site and easy to reach in case of an emergency and available to personally talk to you and your family about your care.
When You're Admitted
Our hospitalist is notified when you are admitted to the hospital, either through your provider's office or the emergency department. You will be seen by the hospitalist in a timely manner based on your medical needs. The same provider will then plan your hospital evaluation, which may include the use of other specialists. Your primary care provider also will be notified at this time.
During Your Hospital Stay
Throughout your hospital stay, we work closely with your primary care provider and medical specialists to ensure your care is excellent and your medical team is well-informed of your progress. The transition from your primary care physician to our hospitalist will be seamless and nearly transparent to you and your family.
When You're Discharged
When you are discharged from the hospital, your primary care provider will resume management of your medical care. We will notify your primary care provider's office after discharge and send a summary of your stay to your provider.
To continue your care with as little interruption as possible, you will need to make a follow-up appointment to see your provider within two weeks of your discharge or sooner if your condition requires more immediate follow up. Your physician can then arrange for any continuing care you may need for your recovery, such as physical or occupational therapy or home health care.
